2/    Please make your payment BEFORE submitting this form. Advise us you are a late booking to retain a table.
3/    The cost is $50 per table for 2 days or 1
4/     Deposit is $15 per table
5/    The BSB number is 063507 (the Commonwealth Bank)
         The account number is 1025 7521 (Ballarat Arms and Militaria Collectors Society)
6/    Please add your name and the word SHOW to your deposit details
